This section would include your plan for the things you would do during this stage in the consultation process. Such things as relationship building, contracting, possible issues you might encounter and your plan to overcome these. (approx. 1 page)
Plan for the Diagnosis Stage in this case
This section would include your plan for gathering additional information, analysis of that data and information, definition of the problem, setting goals and plans to achieve these goals. These are proposed planning activities so in each case there may be some differences in what applies to your case. You should be able to define the problem and set at least one goal for your particular case based upon the available information. (approx. 1 page)
Plan for the Implementation Stage in this case
This section should include your plan for interventions and intervention implementation. Such things as sequence of events, interventions, timelines, individuals implementing and participating in interventions, how the intervention(s) would be carried out would all fit in this section. (approx. 2 pages)
Plan for the Disengagement Stage in this case
This section should include your plan for disengagement. The issues to discuss here would be evaluation of the plan and the entire consultation process (formative and/or summative), how the evaluation would be viewed, analyzed and utilized, and future consultation or collaboration relationships. Your overall termination plan should be mentioned in this section (approx.. 2 pages)
**Note**
The sections on the Stages of Consultation are Plans for each stage and therefore they need to be written in that manner (EX: The Consultant will evaluate the progress towards the Consultation plan goals by surveying the teachers on the student’s progress related to…) This means you should not say the plan worked but give me your ideas for being the consultant in this case in a step by step manner which is informed by the Stages of the Consultation Process.
Conclusion
This section should briefly conclude the paper by concisely speaking to the overall plan and how this fits the case provided. (1 paragraph)
