POSTPARTUM – NEWBORN DAILY CARE PLAN
Patient is 35-year old IRANIAN female who had C-section due to breech presentation of the fetus. Otherwise, no other complications during her pregnancy. Please answer Psychosocial, Cultural Diversity, and Care Plan.

NANDA Requirements:
Submit 3 Priority NANDAs. The mom and baby can be from different couplets however include the birth information if using a different newborn. Please provide 10 interventions, 10 rationale, and 10 patient responses for each NANDA. Please use the NANDAs provided and follow the format below.
1. 1 psychosocial for Mom – Deficient Knowledge or Anxiety
2. 1 physiologic for Mom – Risk for Infection
3. 1 physiologic for Baby – Risk for Impaired Fetal Gas Exchange
Instructions for NANDAs: Use detailed, objective, measurable parameters unique to this pt. that connect through stages of the NANDA.
● Include objective, descriptive information/ data that applies to your pt. specifically. If it could apply to just about any pt. with a similar problem, seek to modify your plan of care so it addresses the needs of your individual patient.
Instructions for completing the Nanda components:- Instructor, please provide examples of higher level Nandas / Problems would be for a pp patient. Other smaller Nandas / Problems = Mini Nandas / Problems.
● NANDA: Using the standard NANDA format. Include pt. specific reasons in the RT section. List pt. data (assessments, labs etc.) in the AEB sections. Note: If the NANDA is “Risk of” – you do not need to list AEB
● GOALS–short and long term: Needs to reflect pt. assessments (not interventions). Time frame should be appropriate for the Dx (not all goals are appropriate for “end of shift” goals).
● INTERVENTIONS: Integrate pt. specifics into each intervention (if it applies to a large number of pts., it is too general, seek to modify each intervention so it applies to THIS pt.).
