THE PURPOSE of this exercise is to begin your initial use of community assessment principles to collect data, determine needs/assets, and consider the health needs of local communities. READ relevant textbook chapters about community assessment and community development. VIEW 3 short videos about community assessment. REVIEW listed CDC document about global health strategies. SELECT a stakeholder in the community to gain information and support in the area identified as a need for the community. You will work directly with this stakeholder to develop, propose implementation and effective evaluation of your action plan. COMPLETE parts 1-3 of this exercise. Type your responses within this table. Each section will expand as you type in your responses. SUBMIT your completed worksheet and action plan. Be sure to accurately follow the directions for this exercise. You will use this assignment during your NUR 433 Global Health Care Practicum course. |
COMMUNITY ASSESSMENT: A community assessment is a broad look across agencies, systems, and community members to learn more about the essential components of their daily lives. It gathers information from varied sources within the community. Examples of community parameters are:
· physical environment · education · safety & transportation · policies & government · health & social services · communications · economics · recreation Community assessments focus on local assets, resources, and activities as well as gaps, barriers, or existing/emerging needs. The process of identifying and appraising this information will help you identify key components in the community and formulate methods of intervention.

Part Two: Community Need or Asset
3. Observe and gather “initial” data about your community. Identify and describe one community need and asset. Identify a member (stakeholder) of the community you will collaborate with to develop your action plan. The stakeholder should be an individual that will be able to provide information and support for the identified need. Stakeholders might be but limited to: a community health nurse, a nurse working in a community health clinic, a city council member if the need is associated with the city, a school board member if the need is associated with the school, school crossing guard if it involves children walking to school, Police officer if related to crime, a Civic Association member if need is related to the neighborhood, Church representative or a shelter if associated with families in need.

Part Three: Global Perspectives
