Health Promotion plan
Create a Health Promotion plan, onan person (but do not post identifying information).
The health promotion plan should contain the following elements:
• Objective and subjective assessment data- this should be very thorough
• Integrate two objectives from Healthy People 2020
o Be sure to include short term and long term goals for each
o 2 Interventions and methods of evaluation for both interventions
4 or more scholarly resources required
