Introduction

This essay will be about the collaborative care given to Mr Jones justify the different skills required for assessment and decisions made by the Multi disciplinary team (MDT) team looking after him, the decision making process, the leadership style and team working will be analysed, the literature surrounding leadership and team working in collaborative care given to Mr Jones.
Scenario
Describe to the reader the key points of the collaborative care decision to be analysed. This section should not be overly long, but it must clarify key parts of the decision in preparation for the analysis that follows.
Mr Jones a 79yrs old gentle man with a past medical history of vascular dementia, prostate cancer and Parkinson’s disease. Mr Jones was in our care for 6weeks at a local hospital. The decision to insert a catheter and put in place a DNAR by the healthcare professionals looking after Mr Jones was not welcomed by the family as they felt inserting a catheter would reduce time healthcare professional spend looking after him and also if a DNAR was put in place their father will not have best care, but this essay will focus on the indwelling catheter….. here you look at literatures on indwelling catheter, UTI, pressure ulcers, why this was done in patient best interest
The decision making process
Using appropriate decision making model critically analyse the decision making process as it occurred within the collaborative care decision. This section must be supported with appropriate theory and literature.
The MDT looking after Mr Jones used the seven steps model of effective decision making while dealing with him, why was the seven steps decision making process used? Write about the advantages and disadvantages, using 2 different literature define the 7steps decision making process, say how it was applied what was good about it ? what was not so good about it ? did it help the situation? Compare with another model, what was the reason it did?. Why was it Ethical for the decision made?. Look at literature about incontinent and sacrum ulcers, UTI in older people, be very explicit as to what point the different model was used in the treatment process support with literature and evidence based practices.
Team working and leadership
Critically analyse the management of issues within the scenario, using appropriate team working and leadership theory and literature.
The MDT involved in Mr. Jones care includes . The nutrition nurse, the speech and language therapist, the tissue viability nurse, the consultant, the social worker, the dementia specialist team and the ward nurses and doctors. In this section we looking at team work and leadership styles and what one was used in Mr Loo case (transformational leadership ) why was it best leadership style? Use literature to support this, what was the outcome for the patient? Compare another leadership style and say why it wont have worked ie autocratic or transactional.
Legal, ethical and professional issuess
You must use at least one of these concepts most relevant to the decision. Your choice should be reflected in your introduction. Once you have decided which of the three concepts are relevant, critically analyse the scenario using appropriate theory, literature and professional guidelines, where relevant.
The ethical, legal and professional issues relating to the above scenario will be discussed and critically analysed, the mental capacity acts 2005, NMC code, right to life, duty of care, prioritising care, evidence based practice, beneficent, and medical paternalism and the four quadrant approach will be considered… this will be elaborated and analysed critically based on the scenario of Mr Jones
Evaluation
Draw your analysis together and give a balanced view on the strengths and areas for improvement of the decision making process analysed above. Also, draw out the most important elements of care improvement in relation to team working and leadership. Finish by identifying, in the final paragraph, key learning points from this process. This should be worded to reflect how your future practice will be influenced by the analysis given above.
This section will be reflecting on what was learnt from the scenario looking after Mr Jones and also how different policies and assessment can affect decision making in nursing, the care of Mr Jones and the importance of collaborative decision making and also the importance of getting updated in the ethical legal and professional decision making process in nursing. All these will be well analysed and referenced in the essay
