12/06/16 3 NUR456: Application of Professional Nursing
Portfolio Rubric, Instructions, and Exemplar
Since your very first class, every course syllabus has advised you to save all your course assignments, as you would need them for
your synthesis course.
That day is finally here. The Live Lecture #2 addresses instructions and expectations for this assignment.
Keep the 10 program objectives in mind when looking through your assignments; you will need to find a minimum of one unique example
for each
objective, which demonstrates growth and achievement. The examples you have will best reflect growth, change, and achievement of
program
objectives. You may repeat examples to demonstrate growth, but every objective will have its own example, so you will have at least
ten appendices.
The following 10 program objectives serve as section headings for each narrative section; therefore, use APA Level 1 header format
(see rule 3.03).
Build your portfolio in the order of the objectives below. Note: copy these objectives exactly; do not modify them.
1. Synthesize knowledge from the liberal arts and sciences and nursing science in understanding global perspectives, stimulating
critical
thinking, and using current technologies.
2. Make sound decisions in the use of patient information and healthcare technology to design, coordinate, manage, and evaluate
nursing
care for individuals, families, and populations.
3. Demonstrate clinical excellence to provide caring, compassionate, and culturally appropriate patient-centered care to people in a
variety of
settings.
4. Communicate, collaborate, and negotiate effectively to improve patient healthcare outcomes.
5. Exhibit professional values and behaviors as affirmed by a code of ethics, professional standards, and legal statutes.
6. Promote healthy lifestyles through health promotion, risk reduction, and disease prevention education for individuals and
populations.
7. Incorporate evidence-based knowledge and theory into nursing practice.
8. Demonstrate leadership and collaboration within interdisciplinary healthcare partners and professional, political, and/or
regulatory
organizations to support change in health care.
9. Allocate and manage resources to ensure patient safety and high-quality health care.
10. Engage in life-long learning and scholarly inquiry to continue professional career development.
Your narrative is your explanation of how these assignments demonstrate your growth and achievement of the 10 program objectives.
This should be a
well-written, focused, concise (nine-page narrative text limit), organized, professional reflection worthy of a JU BSN graduate. You
must give the reader
a solid understanding of the significance of the appendix. Do not just say this assignment helped you communicate better; give
details.
This is a personal reflection, essentially your final exam, to demonstrate meeting all JU BSN program objectives. You must pass this
assignment with
a minimum of 210 points (70 percent). Failure to do so results in failure of the course. Only the narrative portion will be submitted
to Turnitin.com.
Upload the document; do not copy and paste, as the format will change and you will lose APA points.
Here is a sample of narrative for the fourth objective (so all appendices are labeled 4A, 4B, 4C, etc.). As with any sample, it is
not perfect, but will give
you an idea of how to compose your narrative. Look at the APA bubble sheet for correct formatting, using Level 1 heading for all
objectives. Of course,
for APA, the entire paper would be double-spaced. APA format is required (10 percent deduction for not adhering to APA).

Communicate, Collaborate, and Negotiate Effectively to
Improve Patient Healthcare Outcomes
When I first began the JU BSN program in September of 2010, I was lacking in my ability to communicate and collaborate effectively.
In the
spring of 2011, I had my first group assignment in my global health class, titled World Health Organization (Appendix 4A). My
professor asked us to
choose our own groups based on a topic that interests us. Once our group was formed, one of the students took the lead and assigned
us our parts.
We met weekly in a chat room to discuss the project and share our findings. This was the first time I had ever used the chat room to
collaborate with a
group of individuals outside the classroom. I was excited but nervous about how this online group would work. By the time we were
ready to submit our
final project, I was very comfortable communicating with the group, giving suggestions, and voicing my concerns. This class was a
turning point in my
education and professional growth. I realized this once I completed other group projects to see just how far I have come. This past
fall I took Advanced
Critical Care Concepts and was once again asked to work on a group project (Appendix 4B). However, this time the professor chose our
groups, and we
had to choose our leader. Immediately, I volunteered myself. The group obliged, and I took the lead by assigning topics to team
members. As the leader,
I made sure that we had an open communication line and that everyone’s ideas were considered. We communicated weekly either by chat
room or
email and collaborated on the final presentation in a live session.
My last course is a culmination of my achievement for this program objective. Every student had to create a project and select one
project to
adopt to assist with the project development. I selected a project designed to reduce infection rates. The assignment was the
interprofessional response
(Appendix 1C), which we posted to the discussion board. I was able to critically think, problem solve, and research best practices,
which I shared with
the class. The hardest thing about this assignment was providing constructive critique of someone else’s work, but I believe my
example represents my
ability to offer professional feedback and negotiate for change. The project lacked factual infection rates, which I was able to
research and communicate
in a professional manner to the student. I feel I was successful, as she did not get offended by my feedback and incorporated my
feedback into the
project. Every assignment I completed throughout this program played a part in my professional growth because they strengthened my
ability to
communicate and collaborate with others effectively to achieve the task assigned.
This is an example of the appendix from another paper and the projects selected to represent growth in all program objectives. Note:
every objective has
its own unique example. If reused, keep them as initially labeled. The appendix page takes the place of the reference page for this
assignment.
