- Select correct CPT coding principles in accordance with Uniform Hospital Discharge Data Set (UHDDS), National Correct Coding Initiative (NCCI), and American Medical Association (AMA) guidelines to outpatient patient health records.
- Apply correct medical, surgical, radiology, pathology, laboratory, anesthesia, and Evaluation and Management (E/M) codes according to Centers for Medicare and Medicaid Services (CMS) for outpatient patient health records.

Information
- CPT Assistant is the publication that identifies official coding guidelines, clarifies changes, answers questions on coding processes. When official code changes, additions/deletions are made, it appears in this publication. It also provides official positions/answers on questions relating to coding problems
- All hospitals receive a copy of CPT Assistant, or it is incorporated into the encoder software. It is a quarterly publication

- Do not post the actual assignment or instructions.
- Submit only the three CPT Assistant topics in the supplied template.
- Your submission should be formatted as noted below – points deducted for incorrect submissions.
- Put any direct quotes in quotation marks.
- You do not have to identify the reference since the reference is only CPT Assistant
- Do notcopy and paste any CPT Assistant or write verbatim, but summarize your findings, i.e. make believe you are explaining this to someone who does not know anything about coding. No credit for plagiarized work.
- Locate three separate CPT Assistant topics/Subjectsof your choice.
- These can be anything that interest you: procedures, coding processes, guidelines and rules, frequently asked questions (by a specific topic), etc.
- Your submission must follow the required format to receive full credit.
- Format each (all three) CPT Assistanttopics/subject in the same manner.
- Clearly numberyour paragraphs, 1-4, for each case – points deducted for those not in paragraph
form and/or numbered.

- Sign into the AHIMA Virtual Lab – 3M Encoder:

- Click on the tab that states
- Scroll down and click on AMA CPT Assistant
- There are multiple ways to locate a subject within CPT Assistant:
- You could scroll down to the list as noted until you come to a desired topic
- You could click on one of the “alphabetical letters” noted under CPT Assistant on the top of the screen
- You could click on the binoculars located on the topic of the page to perform a “search”.
- We will perform a search utilizing the

- In the search box, type scar revision.

- A list of possible choices should appear.
- Scroll all the way to the bottom of the possible choices, and choose:
