Case Study
You have been responded to a 999 call to attend a woman who is 36 weeks pregnant with her second child. She appears to have lost 1litre of blood per vaginum and is in a collapsed state. You undertake a rapid assessment and note her initial observations to be:
? Pulse: 130 beats per minute and thready. The radial pulse is palpable although it is weak
? Respiration: 24 breaths per minute and shallow
? Blood pressure: 80/40 mmHg
? Temperature: 35.0oC
? Blood glucose: 5mmol/l
? The patient appears pale and only responds to voice.
Her partner tells you that her last ultra sound scan had shown that the cord was presenting below the baby’s head and consequently the plan was for an elective section in 2 days. Prior to the blood loss the waters appeared to rupture and the woman is contracting regularly. Your patient is upstairs in the bathroom. Egress is not simple as there is a 90o left hand turn after the third stair and limited room in the bathroom. The stairs lead to the hall, which is wide enough to accommodate a stretcher and allow for transfer of the patient.
If she gives birth in your care, discuss the consequences for the baby. What would you need to consider?
