NSQH standards : comprehensive care
– Purpose : to identify patient needs and prevent harm. [ (i) meet individual needs and consider the impact of health issues on their life and well being. (ii)risk of harm are prevented and managed through targeted strategies. (iii) developing a comprehensive care plan to deliver a comprehensive care. (iv) minimizing harm by prevent and manage pressure injuries. ]
2nd paragraph – S -> set the scene
74yo/male patient had total hip replacement done, who is high risk of pressure injuries. Hemovac drain insitu post surgery and refused to move due to pain at operative site hence the occurrence of developing pressure sore at the sacral region is high. Patient’s general appearance is skinny and nutrition level is not satisfactory as he lost his appetite due to his current condition. Patient’s main complaint that he feels sore at his sacral region. Upon assessment, discovered that his sacral region has redness and skin breakdown – first stage of pressure ulcer.
3rd paragraph – T-> task
Main issue involved is patient has developed pressure sore post surgery.
It needs to be addressed immediately so that action could be taken to prevent further worsening of the sacral sore.
He would need skin inspection frequently by using the pressure injury risk assessment form and following NSQH Pressure Policy to identify the needs and to plan for strategies for a more comprehensive care plan in order to achieve the desired outcome which is recover from and pressure sore free for the rest of the hospitalization stay as best practice.
One of the main obstacle are engaging the patient with his care changes and ensured patient understand the importance of prevention of pressure sore and gain his trust for cooperation purposes for the betterment of his well being.

Firstly, to inform nurse in charge for the findings of pressure sore and update patient’s care plan
Monitor skin integrity frequently because to check for healing or worsening of sore
Applied extra thin duoderm on that sore area as duoderm act as a protective barrier and a cushion for the skin
Place patient on ripple mattress/air mattress for the purpose of pressure relief for the all bony prominence especially when patient is skinny and not mobilizing much post surgery, and promote blood circulation
Offer and encourage high protein intake and fluid intake if no contraindication with medical history to promote tissue healing
Ensure no tubes/ oxygen probes/ excess linens comes in direct contact with patient’s skin as many cause new sores to appear
Have physio to help mobilize patient from bed to chair and ambulation as this could relief pressure on the sacral region. Besides that, restricted mobility could add complication like urinary tract infection or fecal impaction which can affect the sore further.

What was the outcome? With the continuous of the above actions, there’s definitely healing seen which is a good positive outcome by adhering the best practice and policy.
How did it change things in the workplace? More aware, determine and document likely causal factors of the patient’s condition changes regardless pre or post surgery.
What lessons did you learn from this event? I learnt that it is important to check patient every time especially when patient is transferred back to the ward and when patient is not ambulation post surgery. This is due to their condition are weak or drowsy initially, it is a priority to inspect their skin integrity as they have been lying down on the operating bed for number of hours and may develop sores from operating probes and from the bed itself.
What would you do differently next time? I would notify family members about the finding as this is to keep the in view and aware that pressure sore is common to develop post surgery especially been lying down for too long due to post surgical pain or other reason. The other reason why I would like to notify the family is because the nurse in charge have not done so hence the action of notifying. And would like to take the initiative in notify the pressure sore findings via hospital reporting system with supervision as it was been done by the RN
What recommendations do you have for your future practice if you experienced a similar situation again? Update yourself with the best practice and updated policy to ensure to deliver a competent and quality nursing care to patient.

78yo/female patient refused to shower and was shouting loudly and angrily to me and other patients as well which are sharing the same room. Quite verbally aggressive when being prompted to have a shower before breakfast round. Informed and included the nurse in charge to help persuading patient but patient still refused. Therefore, I took my time to persuade and negotiate with patient to come to terms for a shower.

Main issues involved? Main issue is that patient is refusing profusely to have a shower
What needed to be done?Patient needs to have a shower before breakfast round so that she is ready by the time for doctor’s round.
What tasks needed to be achieved and what was the desired outcome? Patient would need to have a shower as this is encouraged to do every morning shift to start the day for patient herself and health care professionals. Showering also do reduces hospital acquired infection as maintaining self hygiene is important overall and reduces the risk of infections.
What obstacles had to be overcome? Patient is very uncooperative and may not understanding the needs of why showering has to be done at a precise moment.

Effective interpersonal and negotiation comes into the picture
Try to gain patient’s understanding why is she refusing. Be patience and speak in a calming manner in order to gain her cooperation to get the task done.
Explained in a simple and understanding manner the purpose of doing the task
Involved patient in shower preparation and giving her the power to make choices
Negotiate with patient which scented antibacterial wash would she prefer to use by giving her a sense of control over her hygiene aspect
Negotiate with patient in terms like when to have a shower like before or immediately after breakfast and allow it to a certain degree so that the flow of care is not widely interrupted

What was the outcome? patient had a shower and felt good after accomplishing the task. Suggested that would like to continue this routine
How did it change things in the workplace? Aware of the patient’s preference in her care and informing the next shift nurses that will be in to keep the message passed on.
What lessons did you learn from this event? I learnt that not to be so work-task orientated even though it is advised by the hospital to use a timeline to outline the care to be rendered at a specific time for a specific task. I learn that to always include patient in my care as they are my main priority and not the work as it’s a never ending cycle.
What would you do differently next time? I would try to be more understanding with patient’s needs and by understanding more, conflict may be minimized.

As a Registered Nurse (RN) you are required to practice as an RN within the Australian Nursing and adhere to the Midwifery Board Registration Standards and refer to the Nursing Practice Decision Flowchart. Other key responsibilities include: providing comprehensive evidence based nursing care to patients in collaboration with the multidisciplinary team; facilitate and promote patient safety and quality of care; uphold the nursing values of Accountability, Compassion and Continuous Learning and Teamwork.
This case study assessment is designed to aid in preparing you for the transition from student RN to graduate RN. The case study assessment requires you to address the below two (2) selection criteria using case study examples from your current or previous clinical practicums.
Essential Selection Criteria:
- Demonstrates competent, quality nursing care informed by best practice and National Safety and Quality Health Standards
- Demonstrates effective interpersonal, negotiation and conflict resolution skills
When addressing the selection criteria you must use a different example for each criteria. The criteria must be addressed in the STAR format (see below for more information on STAR).
All aspects of each selection criteria must be addressed with appropriate examples given to demonstrate achieving the criteria.
Each selection criteria must be critically analysed using evidence from current literature to support your example. Recommendation/s for future practice must also be included and justified using current literature.

Length: 1500 word count (+/- 10%). You will need to be succinct when writing this case study. Penalties will apply for assessments over the word count.
Grade: Pass/Fail
Due Date: Friday 3rd May 2019 at 2355hrs (WST). The assessment must be submitted via Turnitin. Students are allowed multiple attempts to resubmit until the due date and time.
Some tips for the Case Study:
You will need to read broadly on the topic and provide strong evidence to critically analyse the example and support the justification.
This assessment requires you to critically analyse NOT just describe the examples used
Each selection criteria should be roughly 650 words each
